It will cover the pet(s) that will be staying in the vacation rental property. This agreement is legally binding and is an addendum to the vacation rental lease between both parties.

Pet description and details:

Guest must notify Summit Associates if an additional pets other than the animals listed below will be at the property. Guests must complete the following form for all pets visiting the premises.

Number of Animals (fill out below for each animal):

Per regulations and restrictions:

1. Pet must wear proper identification at all times
2. Pet must be up-to-date on all required vaccinations as per veterinarian and state code.
3. Pet must always be on a leash when not in the home.
4. Pet must be house trained, and the guest will clean up after pet daily.
5. Pets are not allowed in the pool of spa.

Tenant obligations and liabilities:

1. Guests agrees to disclose any and all pets staying at the property. If the guest wishes to have an additional pet, they must get written approval for Summit Associates and will be required to pay additional fees and costs.
2. Guest is responsible to pay for and repair any damage (outside normal wear and tear) caused by the bet both inside and outside the property.
3. Guest will cover the cost of injuries caused by the pet while on the property.
4. Guest is responsible for ensuring the pet is not a public nuisance or disturbance.
5. Guest agrees to pay the additional fees and deposits required in addition to the cost of the vacation rental agreement.
6. Guest agrees to pick up all stool, urine and or vomit and discard in dumpster. Failure to do so will result in a $20 fee per issue that is remaining at the property after check out.

Management rights and obligations:

1. Management will determine which kind of animal, greed and sizes that are allowed or prohibited from visiting the property.
2. Management will determine the number of pets allowed at the vacation rental.
3. Management will not be responsible for injury and or loss of life of the pet staying at the vacation rental.

By signing below, you agree to all of the stiputatio9ns outlined by Summit Associates. Any violation of this agreement could result in eviction, damages and costs or attorney fee costs.
